Intensive Outpatient Program for Substance Use Recovery

Herkey Horizons provides supportive, evidence-based outpatient treatment for individuals struggling with substance use disorders. Our program offers ASAM Level 2.1 services, including group therapy, individual counseling, peer support, and care coordination.

  • Our ASAM Level 2.1 program provides 9-19 hours per week of structured group therapy focused on relapse prevention, coping skills, and recovery support.

  • ASAM Level 1.0
    Designed for individuals who are clinically appropriate for a lower level of care or are stepping down from IOP. Treatment focuses primarily on individualized counseling to support long-term recovery, employment stability, and successful community reintegration. Group therapy may also be incorporated when clinically appropriate and identified in the individualized treatment plan.

  • One-on-one sessions to address personal treatment goals, mental health needs, and individualized recovery planning.

  • Connection to Certified Recovery Specialists and community recovery supports, including mutual aid groups, recovery community organizations, and alumni networks. Peer support is coordinated as part of your individualized treatment plan.

  • Assistance with housing, employment, benefits, transportation, and coordination of care with community providers.

  • Ongoing support and referral coordination to ensure continued stability following program completion.

Intake & Assessment

A comprehensive biopsychosocial assessment and ASAM level-of-care evaluation is completed to determine clinical appropriateness.

Admission to Program

Clients begin structured outpatient services including group therapy, individual sessions, and peer support.

Ongoing Treatment & Support

Progress is monitored through treatment planning, case management, and clinical supervision.

Refer a Client or Request Services

We accept referrals from treatment providers, hospitals, probation and parole, county agencies, and community organizations. Individuals and family members may also contact us directly.
