Herkey Horizons provides supportive, evidence-based outpatient treatment for individuals struggling with substance use disorders. Our program offers ASAM Level 2.1 services, including group therapy, individual counseling, peer support, and care coordination.

Referral or Contact

Clients may be referred by treatment providers, probation/parole, hospitals, or may contact us directly.

Intake & Assessment

A comprehensive biopsychosocial assessment and ASAM level-of-care evaluation is completed to determine clinical appropriateness.

Admission to Program

Clients begin structured outpatient services including group therapy, individual sessions, and peer support.

Ongoing Treatment & Support

Progress is monitored through treatment planning, case management, and clinical supervision.

We accept referrals from treatment providers, hospitals, probation and parole, county agencies, and community organizations. Individuals and family members may also contact us directly.
